  You can make a realistic heart with Adobe Photoshop easily by using Layer Blending options. You can make for example a heart for your special someone with your or their name on it.  
     
  Step 1. Heart Shape  
  Color the background, for example with blue. Then create a new layer.  
     
 

 
     
  To make a heart pick the Custom Shape Tool and select the heart shape.  
     
   
     
  Pick a grey color and draw a heart on a layer you created.  
     
   
     
  Step 2. Hole  
  With the elipse selection tool draw a circle on the heart where the string would go. Hold Shift while drawing to make a perfect circle.

  Now press Delete on the keyboard to empty the hole.

  Step 3. Text  
  Select the text tool and Stencil STD font. Write the words you wish in black.  
     
   
     
  Seleck the letters you wrote by clicking on one with the magic wand tool (make sure you have contigous option off).  
     
   
     
  Now select the layer with the heart and press Delete to empty the letters. Delete the layer with text.  
     
   
     
  Step 4. Blending Options  
  Now for the fun part: we will use three of the blending options: Drop Shadow, Gradient Overlay and Bevel & Emboss. First open the layer's blending options.  
     
   
     
  Select Drop Shadow and set the parameters like it is shown on the picture: 33% Opacity, 3px Distance, 0% Spread and 4px Size.  
     
   
     
  The heart will look like this:  
     
   
  Now select the Gradient Overlay. Double click on the gradient and select two grey colors, one light and one dark grey. Set other parameters like on the pic below: make sure you select reverse, 92% Scale. Also set the angle to -108 degrees.  
     
   
     
   
     
  The third and final blending option is Bevel and Emboss. Select the parameters as they are on the pic below: Inner Bevel for Style, Chisel Hard fot Technique, 1% of Depth, 4px of Size. Select the Gloss Contour that is selected on the pic below and set the Opacity and Shadow mode to 75%.  
     
   
     
  To finish the heart set Image Size to 50% which will soften the edges, and you can also add a string simply with a few brush strokes.
